Wellness Claims (Allstate Critical Illness)

Benefit (yearly)  $100

Pays each calendar year per covered person for one of the policy covered wellness tests. Please review your policy details or review the critical illness brochure located in the critical illness coverage section on this website.

Out Patient  Physician Treatment Claims (Allstate Accident Plan)

Benefit (per visit)  Low Plan - $50 | High Plan - $75

The outpatient physician’s treatment benefit pays when you are treated by a physician outside of a hospital for any reason. Limited to 2 visits per person per year, and 4 visits per year if your dependents are covered. The visit may be provided for a sickness, accident, well exam, physical exam, eye exam or dental exam. Please refer to your policy for the specific benefit information.
